When you visit us for your first appointment you will need to fill out and submit four patient information forms. You may print these forms and fill them out in advance and bring them with you if you wish.

How do I make an appointment?
To schedule an appointment, please call us at (202) 293-3364. You may also request an appointment online by clicking here.

What do I need to bring to my first physical therapy appointment?
-Four required patient forms (print and fill out beforehand – see above)
-Your health insurance card(s)
-A prescription from your referring physician

What happens during my therapy appointment?
At your initial appointment, your therapist will evaluate you and establish a program that will enable you to quickly begin your recovery. You will learn about the treatments you will be receiving, and you will always have the opportunity to ask any questions concerning your treatment.

How long will my therapy appointment last?

The initial appointment will last approximately forty minutes to one hour, depending on the diagnosis. All subsequent appointments will last a minimum of thirty minutes. Please arrive promptly for each scheduled appointment.

What if I need to cancel or reschedule my appointment?
We realize that you may have an emergency occur or unexpected changes to your schedule, but we want to ensure that we respect the time of all of our patients. If you need to cancel or reschedule your appointment, please give us at least 24 hours notice so that another patient may be treated at that time. If you have any questions or concerns about this policy, please let us know.

About Physical Therapy and Rehabilitation

Do I need to see my physician before I can receive physical therapy?
Medicare, Medicaid, and HMO insurance plan patients need to have a referral or prescription from their physician before receiving physical therapy. All other insurance plans by law allow direct patient access to physical therapy in Washington, DC, though a referral or prescription is still strongly encouraged.

How often will I need to come to therapy?
Your therapist will be able to tell you after your first appointment. At that time, the therapist will be better able to evaluate your needs and set up a personalized treatment plan to ensure a safe and effective recovery.

Will my therapist communicate with my physician regarding my treatment?
Yes, your physical therapist will work closely with your referring physician and send him or her regular progress reports regarding your treatment.

Insurance & Payment

Do you participate with insurance companies?
Spinal Balance is a participating provider with 20 government-sponsored and private insurance companies, offering the widest selection of insurance options for physical therapy and rehabilitation in Washington, D.C. As a courtesy to you, we will file your insurance claim with your carrier and receive payment directly from them.

What if I’m paying for physical therapy myself?
Spinal Balance accepts personal checks and credit cards for payment at the time of service. If you would like us to keep your credit card information on file for convenient payment, you will need to fill out and submit a credit card authorization form.
